The Saskatchewan Health Authority has informed SUN that as of April 1, 2023, the SHA will no longer pay for the completion of Statement of Medical Restrictions forms. Employees will be responsible for any costs charged by their healthcare provider for this form. 

The SHA will continue to pay for comprehensive assessments that are coordinated through their office, like the Functional Capacity Evaluations and Independent Medical Examinations.
